Work Environment\n

This role offers the flexibility of a remote work setting, allowing for a supportive work-life balance. The position requires effective communication with clients, families, and interdisciplinary teams consisting of healthcare professionals. You will collaborate closely with colleagues to share information and coordinate care, utilizing various technologies to monitor and assess client needs. While the majority of your work will be performed from home, occasional visits to local community health settings may be necessary to directly connect with clients in-person, ensuring thorough assessments are conducted.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of seniors to identify their emotional, mental, and social needs.

• Develop and implement personalized care plans in collaboration with clients and their families, entirely tailored to individual goals and preferences.

• Provide emotional support and counseling to assist clients and families in coping with challenges associated with aging.
\n
• Act as an advocate for clients, ensuring they have access to essential community resources and services.
\n
• Work alongside a team of multidisciplinary professionals, including nurses and caregivers, to ensure a cohesive approach to client care.

• Conduct regular check-ins and follow-up visits to monitor client well-being and address emerging concerns promptly.
\n
• Facilitate family meetings to enhance communication among relatives and resolve potential conflicts related to a client’s care.
\n
• Educate clients and families about community resources and support groups available to enhance their quality of life.
\n
• Maintain detailed and accurate documentation of client interactions, assessments, and care plans in compliance with legal and ethical standards.

• Keep abreast of current social work practices, policies, and industry guidelines to maintain high standards of care.

Patient Population / Client Focus\n

This position primarily focuses on seniors in our community, many of whom face health challenges, social isolation, or transitional life events. The social worker will provide targeted support to address these unique challenges, enhancing the stability and well-being of older adults, and helping them integrate successfully within their families and communities.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• At least two years of post-graduate experience in social work, particularly in geriatric or community settings.

• Proven understanding of social work principles, aging-related issues, and community resource access.
\n
• Strong interpersonal and communication skills, allowing for effective collaboration with various stakeholders.

• Exceptional problem-solving capabilities and a client-centered approach to care.
