Key Responsibilities\n

• Perform thorough assessments of clients’ psychosocial needs by evaluating emotional, social, and physical aspects influencing their well-being.

• Develop customized care plans in conjunction with clients and their families, tailoring strategies to their individual goals and circumstances.

• Provide emotional counseling to seniors and their families, offering support during challenging times and transitions.
\n
• Advocate for seniors’ rights to access community resources to ensure they obtain essential services, benefits, and support systems.

• Collaborate with an interdisciplinary team, including healthcare providers and caregivers, to create a comprehensive approach to client care.
\n
• Conduct regular follow-ups via phone or video calls to monitor clients’ well-being, address concerns, and adapt care plans as needed.

• Facilitate family meetings to improve communication, resolve conflicts, and unify the care approach among family members.
\n
• Educate clients and families on local resources, available support networks, and programs designed to improve social engagement.
\n
• Maintain accurate documentation of client assessments, care interventions, and progress notes in compliance with ethical standards and organizational policies.

• Stay informed of best practices in social work and relevant industry regulations to enhance service delivery.

Your focus will primarily be on the senior population, addressing their unique social, emotional, and physical needs. Given the increased vulnerability of older adults, your role is crucial in navigating challenges such as health issues, isolation, and end-of-life planning. You will work diligently to offer tailored support that empowers seniors and improves their overall quality of life.

Qualifications\n

• Bachelor's Degree in Social Work from an accredited institution.

• Current and valid state licensure as a Social Worker (LCSW).
\n
• A minimum of three years of experience in social work, particularly with seniors or in community-based settings.

• Strong understanding of principles of social work, especially those pertaining to aging, mental health, and access to community resources.

• Demonstrated ability to empathize, engage, and communicate effectively with clients and families.

• Excellent skills in assessment, counseling, and conflict resolution.

• Familiarity with electronic health record (EHR) systems and other relevant technological tools.

Preferred Experience\n

• A Master’s degree in Social Work, which would further enhance your understanding and capability in this role.

• Experience working within a geriatric population is preferred, ideally in a community health or psychological service context.
