Optimum Care Counseling is seeking a dedicated and skilled Licensed Master Social Worker (LMSW) to join our remote team, providing full-time support to seniors. Work Environment\n

This position is fully remote, allowing you the flexibility to work from home while maintaining regular communication with clients, families, and interdisciplinary team members. You will be expected to engage with seniors through virtual platforms, ensuring that you are accessible while providing high-quality care. Our full-time social worker role requires a commitment to working Monday through Friday, from 8:00 AM to 4:30 PM, with occasional evenings or weekends based on client needs.

Key Responsibilities\n

• Conduct comprehensive assessments of clients’ psychosocial needs, analyzing emotional, mental, and social factors that may impact their well-being.

• Develop personalized care plans in collaboration with clients and their families, taking into account specific goals and preferences to ensure individualized support.

• Provide counseling and emotional support, helping seniors and their loved ones navigate the complexities of life transitions, including health concerns and social isolation.

• Advocate for clients’ rights and facilitate access to community resources, ensuring they receive necessary support services and enhancing their overall quality of life.

• Work collaboratively with nursing staff, caregivers, and healthcare professionals to coordinate comprehensive care and address urgent situations effectively.
\n
• Engage in regular client visits, whether virtual or in-person, to monitor progress, assess living conditions, and address any emerging concerns or safety issues.

• Facilitate effective communication among family members involved in care, helping to resolve conflicts and enhance the support network for the senior client.
\n
• Educate clients on available community resources, support groups, and programs aimed at increasing social engagement and reducing feelings of isolation.

• Maintain thorough and accurate documentation of client interactions, assessments, and care plans while adhering to stringent ethical standards and compliance regulations.

• Continually update your knowledge on current social work practices, ensuring alignment with policy changes and best practices in the field.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Current state licensure as a Master Social Worker (LMSW) in New Jersey.

• A minimum of two years of experience in a social work capacity, ideally within community settings or geriatric care environments.

• In-depth understanding of social work principles, aging issues, and available community resources.
\n
• Exceptional assessment, counseling, and problem-solving skills.
\n
• Strong ability to develop rapport with senior clients and their families, displaying empathy and compassion.

• Excellent communication and interpersonal skills, facilitating effective collaboration with multidisciplinary teams.

• Proficiency with electronic health record (EHR) systems and other relevant computer applications.

Preferred Experience\n

• Experience working with seniors in a community health setting or integrating behavioral health services is highly desirable.

• Knowledge of local community resources, including assistance programs for seniors.

• Experience in navigating Medicare and Medicaid regulations and support services.

Compensation and Benefits\n

The position offers a competitive salary range of $60,000 to $100,000 per year, commensurate with experience and qualifications.
