This full-time position is primarily remote, providing essential support to seniors and their families within a hospice care setting. Work Environment\n

This role allows you to work primarily from home while offering flexibility in your schedule to accommodate the needs of clients. Despite the remote nature of this position, you will be actively engaged with seniors in the community through regular home visits, ensuring they receive timely and comprehensive care. You will collaborate closely with an interdisciplinary team consisting of healthcare professionals, caregivers, and family members to provide a seamless support network.

Key Responsibilities\n

As a Social Worker at Mind Care Therapy Solutions, your responsibilities will include:

• Conducting comprehensive assessments of each client’s psychosocial needs, focusing on their emotional, mental, and social contexts.

• Creating individualized care plans in collaboration with clients and their families to meet specific goals and enhance their quality of life.

• Providing emotional support and counseling to seniors and their families, assisting them in navigating life’s transitions and the challenges of aging.

• Advocating for clients to ensure they have access to critical community resources and services, including healthcare and support networks.
\n
• Engaging in interdisciplinary teamwork, working alongside nurses, caregivers, and other professionals to coordinate care tailored to client needs.

• Visiting clients to monitor their well-being, assess their living conditions, and address any emerging concerns or safety issues.
\n
• Facilitating family communication and mediating conflicts to support the overall caregiving process.
\n
• Educating clients and their families about available community programs, resources, and social engagement opportunities.
\n
• Maintaining detailed and accurate documentation of client interactions, assessments, and progress to comply with regulatory standards.

• Staying updated on current social work practices and policies to ensure ethical care and adherence to organizational guidelines.

This position primarily focuses on working with seniors, who often face complex and multifaceted challenges related to aging and health. You will engage with clients from diverse backgrounds, tailoring your approach to meet the individual needs of each client and their family dynamics. Qualifications\n

• Bachelor's Degree in Social Work from an accredited institution.

• Current state licensure as a Social Worker (LCSW) is required.

• A minimum of three years of proven experience in social work, with a preference for work in geriatric or community settings.

• Comprehensive understanding of social work principles, aging-related issues, and community resources.
\n
• Excellent assessment, counseling, advocacy, and problem-solving skills.
\n
• Demonstrated ability to empathize with seniors and their families, while building rapport and providing compassionate support.

• Strong communication and interpersonal skills to collaborate effectively with diverse teams.

• Proficient in using electronic health record (EHR) systems and other computer applications for documentation and communication.

Preferred Experience\n

While a bachelor's degree in social work is required, candidates with a master’s degree in social work will be given preference. Experience directly addressing the needs of seniors and their families will also be highly valued, particularly in dealing with hospice care settings.

Compensation and Benefits\n

In recognition of the critical work our Social Workers provide, Mind Care Therapy Solutions offers a competitive salary range of USD 80,000 – 100,000 annually. Additional benefits include:
