Clinical Manager For Home HealthSierra Health Care has been the home health provider Las Cruces families have trusted since 1982. As Clinical Manager for Home Health, you'll lead the clinical team that makes that trust possible — overseeing care delivery, supporting staff, and ensuring every patient receives safe, effective, and compassionate care. This is a leadership role for a clinician who is ready to step back from the bedside and invest in the team and systems that make great care happen.Key ResponsibilitiesSupervise, orient, and conduct ongoing competency validation of clinical staff in compliance with Medicare and Medicaid Conditions of ParticipationOversee individualized patient care plans, care conferences, and record audits for regulatory complianceCoordinate interdisciplinary team services, physician collaboration, and internal referralsMonitor quality and compliance through QAPI, infection prevention, and emergency preparedness activitiesManage staff scheduling, clinical resources, and equipment to support effective operationsEnsure accurate, timely clinical documentation in the EMRMaintain availability for consultation, on-call coverage, and direct patient supervision as neededInvestigate and report incidents related to patient care, safety, or employee well-beingOther Duties:Participate in meetings, trainings, and performance reviews as requiredAdhere to infection control, safety, and risk management procedures appropriate to the office environmentDocument relevant communication and administrative activity accurately and in a timely mannerMaintain compliance with HIPAA and all company privacy and confidentiality standardsProvide courteous, timely, and professional customer service to internal and external stakeholdersWork independently with minimal daily oversight and demonstrate initiative in completing responsibilitiesExhibit adaptability to changing responsibilities and flexibility in completing assignmentsDemonstrate consistent and effective use of organizational technology systems and tools required for the role, including communication, documentation, productivity, and data management platformsAdhere to all organizational policies, including job descriptions, mission, and the Employee HandbookCommunicate and interact professionally and respectfully with others to support team goalsPerform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the companyWork EnvironmentPrimarily works in an officeSupervisorExecutive Director - Home Health & Hospice or Director of Home HealthSupervisesRegistered Nurse, Licensed Practical/Vocational Nurse, Certified Nurse Assistant, Physical Therapist, Physical Therapy Assistant, Certified Occupational Therapist, Occupational Therapy Assistant, Speech Language Pathologist, Admissions Nurse - Home HealthRisk LevelOffice-based; limited physical effort or patient contactRequirementsWhat We're Looking ForGraduate of an accredited nursing or therapy program; current, unrestricted New Mexico license requiredMinimum 3–5 years of clinical experience, including at least 1 year in home health or a related settingDemonstrated ability to supervise clinical staff, coordinate care, and maintain regulatory complianceStrong knowledge of Medicare and Medicaid Conditions of Participation and state licensure standardsProven leadership, communication, and organizational skillsEMR proficiency and comfort managing multiple clinical prioritiesValid driver's license and reliable transportationWhy Join UsSierra Health Care is a nonprofit — and that shapes everything about how we lead and how we care.
